CHC Fall Registration Instructions

Start Here ----> You will need a 2011-2012 USA hockey confirmation number.  If not, obtain a number by visiting the link below.

Next ----> Register with our online registration system, one time per season.

You will need to create an account if new to CHC and add players(family) members as participants. If you child(ren) played for CHC, use your family account created last fall. You need to pay the registration fee to complete a successful registration and to be eligible for tryouts and Squirt Skills & Drills.  

Squirt-Midget parents, do not use this link to pay ice fees once you child(ren) are selected for, it will not work.  Only Mites will pay the ice fee via this link.  The Mite session will be Oct thru March.

Last ----> After your player is selected for a team(Squirt-Midget), you will need to login below to fully pay or setup a installment plan.  This the only way for your child to show up properly on Pointstreak for the CCYHL season. It is important to have this step completed prior to your first game.

The payment plan is 25% down, and 25% each month on the 15th, for three months.

Forms and Docs for Tryouts

For each player, you must bring the following forms/docs to tryouts/evaluations:

• CHC Parent Code of Conduct
• USA Hockey Participant Code of Conduct
• Consent to treat
• Waiver of Liability
• USA Hockey 10-11 registration confirmation with bar code(print confirmation)
• Confirmation of registration with CAHA Cares(print confirmation email)

• Copy of Birth Certificate (if you did not play with CHC in 2009/2010)
• CAHA Player transfer (if you did not play with CHC in 2009/2010)

You must print the forms, sign them and turn them in at tryouts. If you arrive at tryouts without forms you will go to a separate check in station and will be charge of $15 to print all forms. Your player will be issued a try out jersey after the paper work has been printed and checked in.

You pay the registration fee once, so if you do not make a team, you do not have to pay again to tryout for another team.

CHC operates closed tryouts.
